Investigation
of Tanker Explosion at Dry Dock CWA investigated the cause
of a multi-fatality incident on board a ULCC whilst undergoing hot
work at a major dry dock and ship repair facility in the Far East.
An independent report was produced for senior management.
Fire Damage to Pulp, Paper &
Forest Industry Products Substantial mitigation of loss was
achieved by supplying an industry specialist to evaluate and
supervise disposal on best possible terms following smoke damage
to a cargo of pulp and paper from an onboard fire incident. The
CWA involvement led to a satisfactory negotiated settlement and
precluded a major claim.
Engine
Room Fire Investigation CWA was instructed to conduct a
detailed post mortem investigation of a serious engine room fire
incident. This required our appointment of a two man team,
comprising our senior fire expert working closely with an
experienced marine engineer. The outcome of this work produced a
satisfactory settlement.
Fire and Explosion
Investigations on Container Carriers
CWA has investigated several fire and explosion incidents on a
range of container vessels, reporting on ignition mechanisms and
fire spread. The CWA team carried out detailed forensic
investigation of the post fire debris, identifying probable
ignition causes. Specialist consultants assisted with the
behavioural characteristics of the cargoes involved.
Fire
Safety Audit and Fire Protection design and Engineering of
Petroleum Installation CWA was commissioned to provide an
independent audit of the existing measures for fire safety and
loss prevention of a strategic petroleum storage, distribution
depot and marine terminal.
The audit examined the existing
facilities for compliance with internationally accepted design and
safety codes as well as evaluating the risk and consequences of
loss of containment of the stored highly flammable bulk liquid
with the potential for serious fire both within and beyond the
battery limits.
A detailed consequence analysis
of failure cases including radiation calculations of pool fires
and full tank fires were utilised.
Recommendations for improvement
in layout and design which subsequently have been implemented by
the plant owners with CWA performing the detailed engineering
equipment specification, tendering and commissioning. This
included the production of computer aided drawings and bills of
material.
Boil
over in Tank Farm Study The Company was commissioned by a
multi-national oil company to carry out an intensive literature
review on previous boil over incidents.
Tank
Farm Radiation Model CWA was instructed by major reinsurers
to review the status of current techniques for risk assessment of
tank farms containing hazardous (flammable) inventories and to
develop improved quantitative methods for evaluating intertank
safety distances in terms of hazardous properties of the
inventory, tank insulation and water deluge. A prototype
computerised model (TANKSAFE) was developed.
Investigation of Serious
Explosion and Fire Incident on Gas Carrier CWA carried out
detailed investigation of a major explosion/fire incident
concerning a semi-pressurised gas carrier, the incident occurring
within the Mediterranean whilst the vessel was discharging a cargo
of propylene. We attended shortly after the incident and advised
on the immediate cause, the emergency response, salvage options,
loss mitigation and loss prevention.
Drilling
Rig Explosion Investigation - North Sea CWA carried out a
detailed technical investigation of an explosion which occurred on
a semi-submersible drilling rig in the North Sea concerning a
failure in a high tension riser. The case required complex
evaluation of high pressure systems, taking account of the
hazardous properties of the fluids involved.
Loss by Fire of Cargo Vessel
Carrying Dangerous Goods CWA was retained to carry out a
detailed post-mortem investigation of the loss by fire in a
general cargo vessel carrying a variety of IMO classed dangerous
goods. The vessel encountered a serious fire during her voyage,
resulting in her eventual loss. CWA formed a specialist project
team comprising a fire expert and a Master Mariner to evaluate the
cause and the efficacy of fire fighting procedures.
Fire and Explosion
Investigation of Tanker Casualty CWA was instructed by
insurers to carry out a detailed investigation of the technical
and operational factors governing the loss by fire and explosion
of a fully laden VLCC. This developed into a major assignment and
involved the formation of a multi-disciplinary team comprising
fire experts, metallurgists, naval architects, tanker specialists
and salvage operators. CWA carried out complex mathematical
modelling of fire spread and oil dispersed from the sunken wreck
as part of our evaluation.
Self-heating
of Direct Reduced Iron Contract research into the root
causes of self-heating of Direct Reduced Iron in the light of
recurring problems in DRI shipments from CIS ports.
